PSYCHOTHERAPY/TESTING FEES
The fees for clients who are not **using insurance **are as follows:
Dr. Natalie Giraldo:
$250 for the initial session
$225 for follow up sessions
Licensed Mental Healthcare Providers:
$200 for the initial session
$175 for follow up sessions
APPOINTMENTS, CANCELLATIONS, AND NO SHOWS Please remember to cancel or reschedule 24 hours in advance. You will be responsible for the entire fee if cancellation is less than 24 hours. Also, if you decide to not attend your scheduled appointment, you will be responsible for the No Show fee. The fee for No Show and Late Cancellations is $80. Due to our high demand after and in order to meet patient need after TWO NO SHOWS or Late Cancellations all future Appointments will be cancelled and you will be placed back on the list.
Cancellations and re-scheduled session will be subject to a full charge if NOT RECEIVED AT LEAST 24 HOURS IN ADVANCE. This is necessary because a time commitment is made to you and is held exclusively for you. If you are late for a session, you may lose some of that session time.
In an effort to be able to provide services to patients ready to commit to therapy we will allow up to Two Consecutive Cancellations. After which we will be unable to hold you weekly appointment y longer. In order to make future appointments you will need to call the office be rescheduled. If your regular appointment time has been filled you will be offered the current available time slots. In the event there is no availability you will be placed at the top of the waiting list for the next available time slot. We apologize for any inconvenience this may cause.
The standard meeting time for psychotherapy is 50 minutes. If you are not using insurance, it is up to you; however, to determine the length of time of your sessions. Requests to change the 50-minute session needs to be discussed with the therapist in order for time to be scheduled in advance.
ACCEPTABLE PAYMENT FORMS
We do not accept any credit cards. We only accept debit cards, checks, and cash as our form of payment.
PAYMENT ON FILE
Payments are due at the time of service. You are required to keep a payment on file with Blooming Mings Psychology in order to pay for any copays, co-insurance, deductibles, or out-of-pocket expenses that accrue during sessions. All cards will be charged when patient statements are received by the insurance and, mailed out to you. Please be aware that you will receive a statement as a receipt by mail if your credit card is charged.
TREATMENT SERVICES FINANCIAL RESPONSIBILITY STATEMENT
It is your sole responsibility to know and understand your insurance policy and coverage. Although Blooming Minds Psychology staff will make every effort to obtain accurate information from your insurance carrier prior to the time of service,** verification of benefits is NOT a guarantee that your insurance carrier will pay for all services rendered.**
It is your responsibility to notify Blooming Minds Psychology of any insurance changes. On each visit there may be multiple services billed to my insurance. You are financially responsible for any copayments, coinsurance, deductible and services not covered through your insurance plan. All payments are expected at the time of service.
TELEPHONE ACCESSIBILITY If you need to contact me between sessions, please leave a message on my voice mail or leave a message with my secretary. I am often not immediately available; however, I will make every attempt to return your call within 24 hours. Please note that Face- to-face sessions are highly preferable to phone sessions. However, in the event that you are out of town, sick or need additional support, phone sessions are available. If a true emergency situation arises, please call 911 or any local emergency room. Please know that any phone conversation over 15 minutes will be charged according to the time spent in increments of 15 minutes. Our fee per session is $225 (Psy.D.) or $175 (LMHC, LMFT, LCSW) . For example: if we spend over the phone 25 minutes, the first 15 minutes will be free; the other 10 minutes will be charged as a quarter fraction of one hour (e.g., 15 minutes) at $75.
***REQUESTS OF SPECIAL LETTERS, SUMMARIES, DOCUMENTS ***
If you request a letter or a session summary to be written outside of our therapy session, or any other document to be produced, you will be charged an hourly fee of $500 per hour. By engaging in these services, you agree not to subpoena Blooming Minds Psychology or request that any staff member provide testimony in any legal proceeding brought by or against you. We do not voluntarily participate in legal proceedings or provide expert testimony. We will only disclose records or information if compelled by a valid court order or as required by law. In the event Blooming Minds Psychology or any staff member is legally compelled to testify or provide records, the requesting party agrees to compensate Blooming Minds Psychology for all professional time, including preparation and travel, at a rate of $500 per hour.
SOCIAL MEDIA AND TELECOMMUNICATION Due to the importance of your confidentiality and the importance of minimizing dual relationships, I do not accept friend or contact requests from current or former clients on any social networking site (Facebook, LinkedIn, etc). I believe that adding clients as friends or contacts on these sites can compromise your confidentiality and our respective privacy. It may also blur the boundaries of our therapeutic relationship. If you have questions about this, please bring them up when we meet and we can talk more about it.
ELECTRONIC COMMUNICATION I cannot ensure the confidentiality of any form of communication through electronic media, including text messages. If you prefer to communicate via email or text messaging for issues regarding scheduling or cancellations, I will do so. While I may try to return messages in a timely manner, I cannot guarantee immediate response and request that you do not use these methods of communication to discuss therapeutic content and/or request assistance for emergencies.
MINORS If you are a minor, your parents may be legally entitled to some information about your therapy. I will discuss with you and your parents what information is appropriate for them to receive and which issues are more appropriately kept confidential.
TERMINATION Ending relationships can be difficult. Therefore, it is important to have a termination process in order to achieve some closure. The appropriate length of the termination depends on the length and intensity of the treatment. I may terminate treatment after appropriate discussion with you and a termination process if I determine that the psychotherapy is not being effectively used or if you are in default on payment. I will not terminate the therapeutic relationship without first discussing and exploring the reasons and purpose of terminating. If therapy is terminated for any reason or you request another therapist, I will provide you with a list of qualified psychotherapists to treat you. You may also choose someone on your own or from another referral source.
Should you fail to schedule an appointment for three consecutive weeks, unless other arrangements have been made in advance, for legal and ethical reasons, I must consider the professional relationship discontinued.
